Note: These 3 zones will spawn mobs which will immediatly attack you. These mobs will be of a level close to your own.

Notes

Frost Swamp, Imirian Ravine, and Oasis of Zaara are 'special' dungeons/zones. You require a quest to enter them on a limited basis (timed). The mobs level inside them are based on the first person in the group to enter it. A nice thread related to this can be found here:

The quests for these can be found one right outside each instance, and 2-3 more at the entrance to the zone the instance entrance is in. (e.g. Just as you enter Poitain)

Villa Quests (Noble Distict)
These scale to your level and are repeatable. (on a timer)

Nice list. Couple things:

30+ - tarantia sewers are actually called the outflow tunnels iirc.
50+ - you missed the cannibal caves

edit: also, I think all of the crafting zones have a frost swamp equivalent, I don't recall the names however.

There's only one place to level from 50-60? That's a little limited, don't you think? I'm level 54 and am down to two quests, which will not ding me to 55. Are there lots more after you hit 55 that I don't see?
