Looses Guild info/Group info

At times especially when leaving a zone My guild potions turn orange and my team / Group info does not update. sometimes /Reloadui fixes issue most of the time I have to do a soft-log to get it working properly.

Cannot effectively run a raid or 6 man if my info does not update properly or loose connection. Any help would be greatly appreciated.
